General Reimbursement Form
*Reimbursement forms MUST be submitted by June 30th of the completed school year. Questions? Email evergreenopt.treasurer@outlook.com.
To receive reimbursement, please download, print, complete and submit to the OPT box in the main office. If you have questions regarding whether or not what you are seeking reimbursement for is eligible, please contact the OPT Treasurer at evergreenopt.treasurer@outlook.com.
Once the form is received, you will receive reimbursement by check within 30 days. If you are a member of the Evergreen Staff, your check will be placed in your mailbox. If you are not a Staff member, your reimbursement will be sent home, in a sealed envelope, with your eldest child.
If reimbursement is of an urgent manner, please contact reach out to the OPT via email.
